Forms Version 2.1

Please consider the following points when printing the forms:

• You need Acrobat Reader 5 or higher to print the forms. Acrobat Reader 4 will show black boxes instead of shades on the top of the page.
• Print the forms always in 100% page size. Here is what you need to do in Adobe Acrobat Reader:
  1. Go to File/Print Setup and make sure you are set
  to the correct paper format:
  
  
  2. Go to File/Print and make sure you set the page scaling to None.
  The zoom factor should show then 100%.
  
  Note: These instructions are for Acrobat 6. Other versions may have slightly different print dialogs.
  
• If you need to make photocopies of the forms, make sure again not to scale them. Also, adjust the forms properly so that they are not skewed. If the margins are not right after making copies, you need to adjust the offset in the settings menu of the Health Age software.
