To run the health expo you will need certain forms for the various stations and certain instructions for those doing the screening; these are listed below and may be printed for your use.
Health Age Forms
The Health Age Questionnaire and Results Forms are needed for every visitor doing the Health Age test. The forms are available in various languages.

Personal Health Record
The first one you need is the Personal Health Record which is given out at the entrance of the expo when you welcome each person attending. This form records their personal results from the various screening procedures. Below are several layouts you may choose from.Option 1

A designed two sided half sheet size (5.25” x 8.5”) printable in color or black and white. The layout is letter size. Print on both sides an then cut. |

Using just the designed record page only, two per page for economical printing. Print letter size, then cut.

Step Test Medical Advisory
